Effective for dates of service on or after December 1, 2004, CPT-4 code 76604 (ultrasound, chest, B-scan [includes mediastinum] and/or real time with image documentation) is a Medi-Cal benefit. This practice is limited to four claims per year, for the same recipient, by any provider.
