On August 1, 2011, TriWest, in conjunction with Wisconsin Physicians Service Insurance Corp. (WPS), is changing the recoupment offset process to the 9-digit tax identification number (TIN) when there are no payable claims at the 18-digit provider identification level. The 18 digit identifier consists of the TIN, the location ZIP code and a sub identification number.

Until further notice, providers should only use the sponsor's Social Security Number (SSN) in their web and EDI transactions with TriWest Healthcare Alliance (TriWest). This includes verifying eligibility, checking referral/authorization status, online claims submission and claims status.

In following TRICARE requirements as well as industry standards, TriWest Healthcare Alliance Corp. (TriWest) requires additional information if unlisted codes or not otherwise categorized (NOC) codes are billed on a claim.

Here is a chart of categories with examples of CPT codes, text description required, and documentation required. If a text note or documentation is required, TriWest prefers that the text is entered on the electronic claim rather than submitting the documentation. Please remember that prior authorization is required for all unlisted codes.

In a continued effort to protect the privacy and security of TRICARE's 9.6 million beneficiaries, starting June 1, 2011, the Department of Defense (DoD) will no longer issue military ID cards with an individual's Social Security Number (SSN) as an identifier.

SSNs will be removed from ID cards and the DoD will issue new cards as individuals enlist or as a card expires. It is anticipated that there will be a four year transition time to issue all new cards.

The West Region Benefit Lookup Tool, available to registered users on the secure provider portal at www.triwest.com/provider, allows you to query whether a service is a covered benefit; whether the service requires a referral or an authorization; and whether the service needs medical review.

There have been issues regarding TRICARE claim denials with a 395 denial code on the remittance advice. Providers are getting this message in error:

This claim cannot be paid until it is billed using the Medicare Code Editor guidelines for procedure code/revenue code combination. Please submit a replacement claim.

TriWest is presenting our latest TRICARE provider education seminars throughout the 21-state TRICARE West Region. In 2011, seminars will be held from April 4 through June 30.
For those new to TRICARE, these seminars provide basic TRICARE information and are a great opportunity to learn about the program. They can also serve as a good refresher for those who have previously attended a TRICARE seminar.
Each attendee will receive the newest TRICARE Provider Handbook and Quick Reference Guides. Updates on the latest enhanced functionality of the secure provider portal will also be covered.

TriWest has updated the comprehensive list of codes on the Prior Authorization List (PAL). The effective date is January 1, 2011. This update added the new 2011 codes that will need authorization and medical review.

All services listed below, provided by TRICARE civilian providers, must be reviewed for medical necessity and require prior authorization for all TRICARE programs administered by TriWest.

For those registered providers who submit referrals and authorizations online on a regular basis, please remember to use a predetermined Request Type profile that includes your requested codes. For your convenience, TriWest has developed a list of over 100 episode of care profiles. When you select a Request Type profile, you will be able to see the procedure codes related to that episode of care. Please use the Request Type profile if the codes you are requesting are within the profile.

In order for TriWest Healthcare Alliance Corp. (TriWest) to make an appropriate benefit determination and understand what is required for a requested service, all services with unlisted codes require prior authorization. In addition, they must be submitted with a description of the service(s).

If a description is not included, TriWest will not process the referral and/or authorization, and return it to the provider. If the code is considered too vague, TriWest may send it back for additional information.

TriWest Healthcare Alliance (TriWest) continues to enhance its offerings to registered users on the secure provider portal. Effective July 14, 2010, TriWest deployed a new reporting tool to assist providers in performing TriWest claims reconciliation and status determination. The Provider Claims Report is available to all users registered on the www.triwest.com secure website and provides claims data for up to a four-week period.

Based on requests from providers, TriWest Healthcare Alliance has improved the view status of online referrals and authorizations on the secure provider portal at www.triwest.com. We have added reason code descriptions that display information regarding a referral or authorization’s status (Approved, Not Approved, Pended or Cancelled).

According to the TRICARE Policy Manual, Chapter 6, Section 1.1, Oncotype DX (S3854) is not a covered TRICARE benefit due to the lack of U.S. Food and Drug Administration status. If CPT code S3854 is submitted, the claim will be denied. Providers also should not submit a claim for this with an unlisted code.

TriWest Healthcare Alliance has implemented some new policies and procedures regarding the use of “Unlisted Codes” to ensure ongoing compliance with TRICARE policy.

Unlisted codes include Current Procedural Terminology (“CPT”) unlisted procedure codes as well as Healthcare Common Procedure Coding System (“HCPCS”) – Not Otherwise Classified (“NOC”), Not Otherwise Specified (“NOS”) and Miscellaneous Supply Codes.
