    1 - Listed are the Federal Travel Reimbursement Allowance Rates. You may enter different reimbursement rates in this table. Years must be listed in ascending order.
    2 - For trips with mileage greater than 20 miles Medicare allows either the "per mile" basis or the "flat fee" basis to calculate reimbursement. In such cases the calculator returns the maximum allowable reimbursement. You may view the alternate reimburement by selecting the check box.
    3 - Enter Trip Date. Must be in D/M/YY format.
    4 - Enter only the mileage associated with the Phlebotomy services. Note: If the trip was not a round trip originating from the lab, do not include the mileage to the first Phlebotomy stop.
    5 - Enter the number of "stops" made to provide Phlebotomy services. DO NOT INCLUDE THE FINAL STOP AT THE LABORATORY.
    6 - If the trip originated from the lab, enter "Yes".
    7 - Enter total number of patients serviced during the trip: Medicare and non-Medicare. Note: Patient count must be greater than or equal to number of "stops".
    8 - Billable dollars allowed for each Medicare patient serviced during the trip.
    9 - Calculation method used for the trip: “Flat Rate” or “Per Mile Rate”.
    10 - Miles for Quantity field entry when using the “Per Mile Rate” calculation. For all "Flat Fee" calculations the Quantity field defaults to "1".
    Buttons:
    UPDATE - recalculates entire spreadsheet based upon current input values
    RESET - clears all input entries and restores all default values and settings
    PRINT - Prints spreadsheet to default printer
