Complete Blood Count Billing Errors

Noridian Administrative Services has noticed an increase in physician and physician office coding errors for Complete Blood Count (CBC), with and without differential, and other laboratory test billing.

The most common billing errors identified:

• Billing a CBC with differential when the differential was not specified in the physician order or supported by his/her chart notes.
• Specific lab test(s) were not documented in the physician notes.

HCPCS/CPT	Description
85025	Complete Blood Count, with differential WBC, automated
85027	Complete Blood Count, automated

The requisition must match the physician ordered test(s) indicated in the chart note.
