Providers may bill definitive qualitative and quantitative testing on a maximum of seven drug classes per patient per date of service. If the physician orders more than seven drug classes (either definitive qualitative or quantitative) on a patient per date of service, submit with CPT code 84999, enter CSPAN and the number of drug classes in the SV101-7 field. Remember to use the precise text ‘CSPAN-Number.’ Do not add spaces between the text and hyphen or the hyphen and the number of drug classes.
