UPDATED: Claim Filing Guidelines for New CMS ‘X’ Modifiers

The communication originally posted on January 5, 2015, is being reissued on February 12, 2015, to clarify that claims reporting modifier 59 should only include an X modifier (if applicable) when reporting services for members enrolled in Blue Cross and Blue Shield of North Carolina (BCBSNC) commercial products. Claim submissions for members enrolled in BCBSNC’s Medicare Advantage products (Blue Medicare HMO^SM and Blue Medicare PPO^SM, should continue to report modifier 59 (when applicable) and not include X modifiers at this time.
