Allergen Specific IgE Testing: Updated Documentation Criteria

Effective for dates of service on or after January 1, 2015, coverage criteria for CPT code 86003 (allergen specific IgE; quantitative or semiquantitative, each allergen) have been updated. Manual review is no longer necessary. Without justification of medical necessity documented in the patient’s medical record, claims billed with code 86003 may be denied.
