Comprehensive Error Rate Testing (CERT): Significant Increase in Pathology and Laboratory Service Errors

CMS implemented the Comprehensive Error Rate Testing (CERT) program to measure improper payments in the Medicare Fee-for-Service (FFS) program.  Recent CERT data analysis continued to identify an increase in pathology and laboratory service errors.  Pathology & Laboratory Services contributed to approximately 29% of CERT errors. Errors received for incorrect coding, insufficient documentation, failure to demonstrate medical necessity with a signed physician order, and response received-improper documentation.
