Correction: coverage comments for some Drug Testing codes

BCBSMI 2015 HCPCS document, listed several codes as “Covered by BCBSM.” However, the following codes should have been listed as “Requires manual review” as of Jan. 1, 2015: 80304, 80323, 80338, 80339, 80340, 80341, 80342, 80343, and 80344. These codes require a complete description and an itemization of charges and will be manually reviewed for payment consideration.
