Drug Testing CPT codes 80300-80377

To align with CMS, UHC will no longer reimburse Drug Testing CPT codes 80300-80377 until more definitive direction is received from CMS on how these codes should be paid. The change to this policy will become effective with claims for DOS on and after 6/1/2015.
