Extension of Revised Electronic Funds Transfer (EFT) Authorization Agreement (CMS-588)

CMS issued JSM/TDL-10252 indicating prior versions of the EFT Agreement could be received and processed through January 1, 2011. CMS is extending the time providers and suppliers may submit the prior version EFT form. Providers and suppliers may begin using the revised EFT form that is currently posted on CMS’ Web site, although contractors will continue to accept the prior version of the EFT authorization agreement until June 30, 2011. The necessary changes have been made to the Provider Enrollment, Chain and Ownership System (PECOS) to incorporate the revised version of the form effective April 2011.
