Lab Frequency Editing

Effective 07/01/2014, BCBS Georgia will add a frequency limit of once per date of service for the following CPT and HCPCS codes:

•  81479 Unlisted molecular pathology
•  36415 Collection of venous blood by venipuncture
•  36416 Collection of capillary blood specimen
•  S9529 Routine of venipuncture for collection of specimen
