Laboratory Authorization Code List

The Laboratory Authorization Code List has been updated as follows:

·        Effective DOS through 12/31/12:

·        Heavy metal screening (CPT codes 83015 & 83018) no longer requires PA

·        Codes 83890-83909 are discontinued

·        Effective DOS 1/1/13 and forward, report BRCA testing with CPT codes 81211-81217

·        Effective 3/31/12, S3818, S3819, S3820 and S3822 are discontinued
