- Merged relevant information from the Medical Policy Manual into this module
- Updated the introductory note box and updated the Introduction section
- Added CLIA certification types in the Clinical Laboratory Improvement Amendment Regulations section and removed the table listing provider types subject to CLIA rules
- Added a note box explaining the HAF exception for outpatient laboratory services in the Reimbursement Methodology for Laboratory Services section
- Added modifier descriptions in the Billing Procedures for Laboratory Services section
- Renamed the Consultative Pathology Services section and removed the corresponding code table and billing guidelines for codes that allow interpretation
- Added a reference to cervical cancer screening and added a note box regarding PA requirements in the Policies and Procedures for Specific Laboratory Services section
- Added information about covered HIV testing methods in the HIV Testing section, and removed specific USPSTF guidelines
