Although a predetermination is not required, Anthem encourages providers to obtain one prior to performing any of the following CPT codes: 80100, 80101, 80104, 80102, 80103, 80154, 80184, 82055, 82101, 82145, 82205, 82491, 82492, 82520, 82541, 82542, 82543, 82544, 82646, 82649, 82654, 82742, 83805, 83840, 83925, 83992, 84022 and HCPCS codes G0431, G0434.
