MolDX Excluded Tests

Through the MolDx process, specific genes/gene components were determined to be excluded tests and will be denied. If literature becomes available to substantiate the results on a specific gene are used in the management of a beneficiary’s specific medical problem, interested stakeholders may submit an Excluded Genetic Test Reconsideration Request as outlined in the MolDX Excluded Test Reconsideration Process.
