Preferred Medical Laboratory (PML) Providers Drug Screening Billing Changes

Effective November 1, 2012, drug screening laboratory services should be billed with HCPCS codes G0431 and G0434 to be eligible for reimbursement. Current CPT codes 80100, 80101 and 80104 may be billed, but they will not be separately payable by Blue Cross and Blue Shield of Alabama.
