Reopenings for Minor Errors and Omissions – Utilizing the Large Various Adjustment Macro Process

National Government Services Telephone Reopening Unit encourages providers to use the LVAM process when submitting a request for correction of 25 or more, for the same or similar situation. The LVAM process is a more efficient way to adjust several claims with the same or similar situation. Examples of like services that can be corrected via the LVAM process are; changing modifiers or procedure codes, adding diagnosis codes, increasing billed amount, changing the place of service, and changing the quantity billed. Simply complete the Part B Reopening Request Form, and attach the LVAM Form. Using this form will ensure the correct claims are identified for the adjustments being requested, it is quick and efficient, and prevents NGS from contacting you for further clarifications.
