Vitamin D policy change
February 28, 2014The Vitamin D Testing medical policy is being revised. These changes will be effective for dates of service on or after May 1, 2014 and apply to group and Individual products, excluding Medicare Advantage plans. Diagnosis codes 274.11 and 588.81 were added to the existing list for CPT 82652 Vitamin D; 1, 25-dihydroxy, includes fraction(s), if performed. Effective May 1, CPT 82306 Vitamin D; 1, 25-dihydroxy, includes fraction(s), if performed, will be considered to be medically necessary ONLY when billed with specific diagnosis codes in the policy.
