MolDX Claim Submission Instructions Updated

All claims for molecular tests in the MolDX code ranges require additional information for processing per the Molecular Diagnostic Testing policy. In addition to the proper CPT code, the claim line must contain the test identifier (DEX Z-Code™) associated with that test.  The tests subject to this requirement include two microbiology code ranges.  Effective September 1, 2017, CPT codes 87631-87633 (Infectious agent detection by nucleic acid, respiratory virus 3-5, 6-11 and 12-25 targets, respectively), and 87149-87150 (Culture, id by nucleic acid direct and amplified probe technique, per culture or isolate, each organism probed, respectively) will require the use of a DEX Z-Code™ for Part A and Part B claims submission.
