Alcohol Laboratory Test Becomes Benefit

Effective for dates of service on or after October 1, 2015, HCPCS code G6040 (alcohol [Ethanol]; any specimen except breath) is a benefit. The code may be billed with modifier QW (CLIA waived test). The code must not be reported in conjunction with CPT code 80320 (alcohols).
