Clarification Regarding the Discontinuation of the Advance Notice for Non-Coverage Form
November 26, 2014Effective Dec. 1, 2014, UHC is making additional changes to the Charging Customers for Non-covered Services Protocol in UnitedHealthcare’s Physician, Health Care Professional, Facility and Ancillary Provider Administrative Guide with respect to Medicare Advantage members only. They want to clarify that even though the Advance Notice of Non-Coverage (ANN) form has been discontinued, you must continue to include the GA modifier on the claim for non-covered services to indicate that you have met specific requirements under the revised Charging Customers for Non-Covered Services Protocol. The GY modifier (item or service statutorily excluded or does not meet the definition of any Medicare benefit) no longer needs to be included on claims for non-covered services provided to Medicare Advantage members.
