Denials of Family PACT Claims for Chlamydia Trachomatis and Neisseria Gonorrhoeae Testing

Family PACT claims for CPT codes 87491 (Chlamydia trachomatis, amplified probe technique) and 87591 (Neisseria gonorrhoeae, amplified probe technique) are being erroneously denied with Remittance Advice Details (RAD) code 0621: The monthly/yearly limit for this procedure has been exceeded. The claim is denied. The Department is aware of this issue and is working on a resolution.
