Effective November 15, 2017, BCBSAZ is implementing two changes in requirements for claim adjustments

Claim adjustments must be submitted with all line items from the original claim so that the claim information is complete and can be compared line by line to the original. The adjustment will be returned if the claim information is incomplete. Think of it as submitting a new claim that has all the correct information on it. There is a limit of no more than two adjustments to the original claim. Any claim submitted for a third adjustment will be returned. To avoid multiple adjustments, ensure that you have entered accurate codes and charges before submitting all claims and adjustments.
