Family PACT Code Conversion Final Policy: S -codes discontinued 12/30/13

Effective for dates of service on or after December 30, 2013, Family Planning HCPCS Level III local codes will be discontinued. The local codes will be replaced with HCPCS Level II national codes. The following S-codes will be discontinued.
