IHCP will no longer cover chemistry and toxicology tests
April 27, 2017Effective April 28, 2017, the Indiana Health Coverage Programs (IHCP) will no longer cover the following HCPCS codes. This change applies to all IHCP programs for dates of service (DOS) on or after April 28, 2017.
