Incorrect Death Terminations in Social Security Administration Records

CMS has learned that the Social Security Administration (SSA) processed incorrect death terminations for over 3,000 beneficiaries on August 10 and August 11, 2016. Additionally, SSA processed incorrect death terminations for over 300 Rep Payees, resulting in suspension of the beneficiary represented. SSA is working to correct these records as quickly as possible. SSA is preparing a notice to send to all affected beneficiaries.
