KRAS Pathology Submission Requirement Change

UHC has announced the following changes regarding KRAS Pathology Requirement submissions to obtain coverage for ErbituxÆ (cetuximab) and VectibixÆ (panitumumab): • UnitedHealthcare has removed the requirement to submit a pathology report. This change occurred with our system update and was effective for claims submitted after Feb. 21, 2011 • Oxford Health Plan has eliminated the requirement for a pathology report submission for claims submitted after April 1, 2011 • The River Valley entities eliminated the requirement for a pathology report for claims submitted after April 1, 2011
