All MolDX laboratory services receiving claims rejections between 8/27/2015 and 9/3/2015 with either of the reasons listed below on or between the dates of August 27, 2015, and September 3, 2015, will need to resubmit these claims due to an error in the claims system. Please resubmit these services as a new claim.
