Laboratory Services Manual Updated

The Laboratory Services Manual is updated and may now be found in the Physician Services Manual. The Laboratory Services Manual will be archived effective July 1, 2017. Specific coverage on CPT or HCPCS codes may be found in the Utah Medicaid Coverage and Reimbursement Code Lookup.
