Medical Records to the CERT by .tif and/or .pdf Formats

November 3, 2009

This article educates providers on the submission of requested medical records for a Comprehensive Error Rate Testing (CERT) review. Previously, all medical records requested for a CERT review were paper copies faxed or mailed directly to the CERT Documentation Contractor (CDC).

In addition to paper faxing or mailing, CERT is now allowing and encouraging providers to submit medical records electronically on CDs. The CERT can receive images in both .tif and .pdf file formats. By submitting records electronically, providers can save money in copying and postage and reduce the turnaround time of receipt.

Please adhere to the following directions when sending by .tif or .pdf:

  1. Send the specific documents listed on the Bar Coded Cover Sheet to support the services of each claim identified on the Medical Records/Documentation Pull List.
  2. Place the bar coded cover sheet in front of the medical records/documentation being submitted for review.
  3. Make sure all pages are complete, legible, signed and include both sides and page edges where applicable.

Mail the medical record documentation or CD to:
