New Forms for Reopening Request coming soon

CGS will be updating the current Reopening request forms for J15 Part B. Once the new forms are available, you will no longer be able to use the current forms you are using to request a reopening. Please remember to delete any saved forms you have in your files, and start using the new forms immediately from https://www.cgsmedicare.com/partb/forms/index.html.  Old forms will not be accepted and if used can cause a delay in accepting your request. To make this transition successful Cigna has removed the Reopening guidelines from the Appeals/Redeterminations section.  Reopening guidelines now has a separate location in the Appendix section at https://cgsmedicare.com/partb/index.html .
