Prior Authorization Requirements For Genetic Testing

Effective April 1, 2016, all genetic testing codes in the CPT range 81201 – 81479 will require prior authorization.  Genetic testing codes in this range that are covered for Wyoming Medicaid will require specific documentation to be included with the prior authorization.
