TAR Policy Updates for Genetic Tests for Glioma CPT Codes

Effective for dates of service on or after October 16, 2015, Treatment Authorization Request (TAR) criteria for genetic testing for gliomas has been updated. Providers are reminded that TARs for CPT codes 81402 and 81403 must include one of the documentation requirements listed in the Pathology: Molecular Pathology manual section. Testing for Telomerase Reverse Transcriptase (TERT) gene in patients with glioma may now be billed using CPT code 81479 (unlisted molecular pathology procedure). A TAR is required with documentation that the patient has a diagnosis of grade II, III, or IV glioma.
