UCare’s 2020 Authorization & Notification Grids – Medical Services 2020 Changes

The following changes were made to the authorization and notification requirements for UCare’s State and Federal Programs (UCare Medicare Plans, UCare Medicare Plans with M Health Fairview and North Memorial, EssentiaCare, MSHO, PMAP, MnCare, MSC+, UCare Connect, UCare Connect + Medicare):

2020 Changes:

• Genetic/Molecular Diagnosis tests – removed Oncotype DX Breast testing (81519) from Authorization requirements. 
• Service name change – changed “Vagus Nerve Stimulation” to “Cranial Nerve Stimulation including Vagus Nerve.”
