Policy Title	Effective Date	Policy Summary
Outpatient From-to Date Policy, Facility*	November 1, 2021	The new Outpatient From-to Date Policy, Facility will be effective for dates of service on or after November 1, 2021 Consistent with SMS and NUBC guidlines, each revenue code on an institutional claim for outpatient services must include a line-item date of service.  The line-item date of service must be within the from-to date on the claim
