Unitedhealthcare Now Requires Certain Corrected Claims To Be Submitted as Original

XiFin’s Electronic Data Interchange (EDI) team has identified a change in how UnitedHealthcare (UHC) is processing corrected (repaired) claims that may impact your billing workflows. This change only applies to diagnosis code and procedure code corrections for UHC.

UHC is now rejecting certain corrected claims submitted as replacements or voids and requires these claims to be resubmitted as Original claims instead. UHC is denying these submissions with Reason Code A3-21, accompanied by the following message: “Repaired claims should be sent with the original frequency code, not a replacement or voided indicator.”

If you are correcting claims for UHC, particularly diagnosis or procedure code corrections, please adjust your workflow immediately:

  • Do not submit corrected claims to UHC as replacements or voids
  • Resubmit corrected claims as Original claims to avoid rejections
  • Review any automated or manual correction processes to ensure they align with this requirement

We have seen this message returned both on its own and in tandem with other rejection messages, including those related to diagnosis or procedure code issues. Submitting these corrections as replacements will continue to result in avoidable denials.
