UnitedHealthcare to Enforce Stricter Reimbursement Rules on Routine Lab Tests Beginning December 1, 2025
September 5, 2025Starting December 1, 2025, UnitedHealthcare (UHC) will implement a new Routine Laboratory Testing Policy that introduces automated pre-payment edits for professional and facility claims in office, hospital outpatient, and independent lab settings. These edits will enforce frequency limits and diagnosis-to-test alignment, with claims denied before payment if they do not meet policy criteria. Emergency, inpatient, and observation services are excluded.
The policy impacts a wide range of commonly ordered tests, including A1c for diabetes, iron studies, pancreatic enzyme tests, prostate biopsy analysis, flow cytometry, and fecal biomarkers. Industry experts caution that the changes may drive higher denial rates, requiring providers and labs to be more exact with documentation and coding to secure reimbursement. Providers are encouraged to review UHC’s guidelines and adjust workflows to mitigate potential revenue loss.
