Update to Screening Mammography Reimbursement Restrictions

Effective for dates of service on or after March 1, 2016, in accordance with United States Preventive Services Task Force guidelines, reimbursement for screening mammograms is restricted to females 50 through 74 years of age with a frequency limit of one screening every two years. There are no diagnostic restrictions for screening mammograms. A Treatment Authorization Request (TAR) may override age restrictions. A TAR will override gender restrictions.
