Updated TAR Requirements for Molecular Pathology

Effective for dates of service on or after November 1, 2015, CPT codes 81250 (G6PC [glucose-6-phosphatase, catalytic subunit][Glycogen storage disease, Type 1a, von Gierke disease] gene analysis, common variants [R83C, Q347X) and 81287 (MGMT [0-6 methylguanin-DNA methyltransferase] [glioblastoma multiforme], methylation analysis) will require a Treatment Authorization Request (TAR) for reimbursement. The documentation previously submitted with the claims must now be submitted with TARs.
